Perineal Groove: A Rare Perineal Malformation

Abstract

Aim: To our knowledge, there is no published large-scale clinical trial on the congenital perineal groove, there are few cases were published in the literatures. This case report describes a 1 year-old female infant with complains of the perineal wet sulcus (like fresh wound), the perineal sulcus was found to be extended from valvular genitalia to the anal opening. The mother noticed this groove at the age of one month.

Presentation of Case: The infant was seen by many doctors, medication were administered but no improvement. A local  examination of the infant revealed an abnormal wet defect, non tender, about 1.5cm in the perineal area, red in color and extended vertically from valvular genital anteriorly to the anal  orifice posteriorly. The physical examination revealed a congenital perineal groove. After counselling of the parents, surgical excision has been done, due to repeated infection, persistent mucus discharge, and cosmetic purpose. No any complications or recurrence. Perineal groove is a rare entity. It seems to be extended vertically from the vaginal fourchette anteriorly to the anal opening posteriorly. The diagnosis of the defect is set mainly on the clinical assessment of the lesion. In most of cases, the defects are treated conservatively with some indications for surgical intervention.

Conclusion: The defect usually treated conservatively, but repeated infection, persistent mucus discharge, and cosmetic purpose are good indications for surgical intervention. The perineal groove should be one of differential diagnosis of perineal anomalies.
