Abstract
Acute thoracic syndrome is one of the specifically rare and serious pathologies in homozygous sickle cell disease. Diagnosis should be suspected during pregnancy in febrile or non - febrile patients with vaso - occlusive painful crisis associated with severe respiratory distress without obvious cause as shown by this 32 years old pregnant woman. The treatment must rest mainly in sub-equipped environment on blood transfusions, antibiotic therapy, oxygen therapy and especially apheresis of red blood cells
